1Assesses and evaluate patients /clients status by:
AEstablishing and implementing the plan of care BRegularly re-evaluating patient/client needs CReviewing and revising the plan of care as necessary at least every 60 days2Specifies in writing, the assignment for Home health AidesSupervises the HHA every 14 daysReviews and revises assignment plan as necessary to meet the patients needs at least every 60 days3Supervises LP(V)Ns at least every 60 days (every 30 days in Indiana) or as required by patients needs4Complies with accepted professional standards and principlesFollows all rules/regulations in home care as mandated by state/federal regulations-Oregon State Specific:
Perform a LPN supervisory visit every 60 days when the patients condition is stable and predictable, and at least every two weeks when the patients condition is critical and fluctuating-Wisconsin State Specific:
